I agree to abide by the rules and regulations set out by Alpha Gym and the use of our facility
I agree to ;
⦁ Membership payments cannot be refunded at any time, nor be extended or covered due to lost time for any unforeseen circumstances
⦁ To Sign/Scan in at reception upon arrival of every session
⦁ Put away any equipment i.e weights, accessories that you have used
⦁ Lower weights in a controlled manner, no dropping or slamming weights/machines
⦁ I have consulted/been authorised by a doctor before undertaking and form of exercise and have been deemed fit and safe to use the gym
⦁ I fully understand that there may be risk of injury to myself when using free weights or any gym equipment. If I should receive any form of injury, I will not hold Alpha Gym or any member or owner responsible
⦁ I confirm that the information contained in this document must not be under any circumstances be made available to any third party or organisation without our consent
⦁ I confirm that the details I have provided are true and accurate to the best of my knowledge
GYM INDUCTION WAVER
All new members are advised to attend a free gym induction which will show you how to safely use the
facility and all machines. However, should you feel you do not need this service, please complete the gym
induction waiver declaration below in the health questionnaire section - this will be listed under PARQ on mobile app.
I understand that exercise can be physically demanding and if performed incorrectly can cause serious
harm. I have opted to not attend the gym induction offered to me and therefore assume all liability for any
possible injury caused by the exercises I choose to perform. I acknowledge that I am physically fit and free
from any illnesses that may be aggravated by performing exercise. I am a regular user of gyms and I am
familiar with most gym equipment.
I am aware that if in the future I decide that I do indeed need to be instructed in the use of the facility or
machines, I will always have the option to book an induction free of charge, based on availability.
